The
Homecoming Services Program assists frail seniors returning
from hospital care
to their central San
Francisco homes.
Often, seniors face this difficult
transition alone. In contrast, we connect seniors with temporary and ongoing
supportive services to ease them back to independent and healthy living.
The program's
partners include San Francisco Senior Center (providing temporary case
management), St. Francis Memorial Hospital and
St. Mary's Medical Center
(providing discharge planning),
Little
Brothers Friends of the Elderly (providing volunteer medical escorts),
In-Home Supportive Services Public Authority
(providing in-home personal assistance),
and Project Open Hand (providing
groceries).
Services are free of
charge to low-income clients and are based on a sliding scale, relative to
income, for others. The services are made possible with the generous help of private
donations from people like you and
through public-sector donations from organizations such as the
Richard and Rhoda Goldman
Fund, The San Francisco Foundation,
Wells Fargo,
the San Francisco
Department of Aging and Adult Services, and others.
